The game was said to be controversial and was published by Sega.

Upon the take over of The Organizers a terrorist group that made the whole city of Virrigan to be a game show known Death Watch the game starts. Citizens were trapped and were forced to join the game show. They have no choice but to fight for their survival against unknown enemies and win a one hundred dollars cash prize.

A dominant character of the game known Jack will be the one that players control. Jack will be the one to battle around the city viciously with different objects like chainsaw, street signs, bats, rose thorns, and daggers that can be found in the environment. The more creative, bloodiest, violent kills will take Jack further in the game and have a higher chance to survive.

The presentation of the game has only three colors graphics and as it was after the graphic novel Sin City. The designed graphics were in high contrast of black and white colors and a red when blood comes out in fighting. Shigenori Nishikawa told that they wanted MadWorld to be seen in a comical way despite its dark tone and extreme violent feature.

Even before the release of MadWorld it has been given a lot of mixed reviews and controversies had been rising. In United Kingdom Madworlds release has been questioned due to the highly violent content of the game. These controversies also affect other target places for the release.

But though controversies had been thrown to the game, Nintendo still supported the game. The release would be release early this year and the target places were North America and Australia. The decision whether to release the game in Japan or not is still on hold.
